Codex trying to read from non-existant Bashes in Claude

I have been getting this error from Codex in Claude Code where it will try to read the output from a non-existent Bash:

Is it possible to prevent this from happening in any way? Maybe through System Instructions or any other way

Maybe you could add some user prompts in your CLAUDE.md, like:

Don't call BashOutput tool directly, if you want to call a bash command, please call Bash first, then you can call BashOutput to get the output later.
