Extern types V2 RFC #211

Summary
I'd like to attempt to resolve as many of the unresolved questions on the extern types RFC as possible, in order to get it on track for FCP, especially as it includes making 2024 edition changes. It may also be possible to split out a smaller subset of it if necessary to get the edition sensitive pieces in first.
