feat: Integrate documentation map into agent workflow

- Add documentation map section to main CLAUDE.md
- Update role CLAUDE.md to check for documentation map first
- Add documentation map loading to _core.md workflow
- Include documentation map context in dev role
- Auto-load documentation map in conductor context file

This ensures agents are aware of and use the valuable project
context created by the [INIT] discovery task, improving their
understanding of the codebase when working on tasks.
