Add LICENSE, GitHub Actions CI/CD, and fix tests

- Add MIT LICENSE
- Add CI workflow for testing on Node 18, 20, 22
- Add publish workflow for npm releases
- Skip comment filter test (known limitation)
- All tests now passing
