Refactor and add additional cases to SourceMapperTest

- Created a static factory method for `Source` to remove the redundant
  constructor arguments.
- Created a static helper method to resolve the fixture path.
- Test prefix inclusion and exlusion
- Test suffix inclusion and exlusion
