Laravel Validation

I'm trying to not allow any duplicated form data in my database, i used the rules like it says on here "https://skydiver.github.io/october-plugin-forms/docs/validation/validation/", but it's not working, so i tried using the key as "form_data" and the value as "unique:martin_forms_records", but it's still allowing duplicated submits.
I would REALLY appreciate some help here.. Thanks in advance
