Posters: One or two posters?

In general, when "Include Poster Image" is selected, it will return the poster image, and then beneath it, the notification text as laid out in your code.

However, sometimes, it returns all of that, plus it adds another poster, as though quoted within the text.

Why's that? What differentiates this returned notification, and is there a way of preventing it from happening?
