Adding Support for Model Context Provider tools for OpenContext (#221)

Solve Part of[ Linear
issue](https://linear.app/sourcegraph/issue/CODY-4572/support-all-resources-and-tools-configured-in-mcp-via-openctx#comment-f42ee177)

NOTE: While there are many potential directions this project could take,
for now, we need a reasonable implementation using the current open
context protocol. Once this implementation is merged, any subsequent
changes to this code are expected to be minimal. Most future updates
will likely involve changes in the Cody repository rather than here.
I’ve tested this thoroughly, and it works as intended. If needed, I can
share specific testing examples, but I believe this is ready for
approval and should be merged
