Multiple images from default collection are attached to the same model instance

I am facing a random problem where multiple images are being attached to the model instance when only 1 media item is uploaded from the request.

• Laravel version: 8.79
• Media Library version: 9.11.1
• Php version: 8.0.14
• Deployed using nginx with php-fpm

ProductsController:
