registerMediaConversions doesn't always generate

Anyone had an issue where the queue processing had no error, however, not all thumbs/previews were generated (I selected several images at once using the Livewire 3 component)? I uploaded 5 images, but only 2 generated previews and thumbs. No log errors, no failed jobs, no queue issues, always says running and done for each image.
