Better document monitoring

Improve documentation of the monitoring solution, including an architecture diagram of Prometheus and its exporters, and of the EFK centralised logging.

Explain the labels applied to Prometheus metrics, including the difference between `instance` and `hostname` for `openstack-exporter`.

Explain how to silence alerts when a hypervisor is down.
