Clarify format of "id" header in SUBSCRIBE/UNSUBSCRIBE

The spec has no information about the format of the "id" header:

• Must it be an integer or can it be alphanumeric?
• Which characters are allowed, and are there any length limits?
• Is there a minimum length an implementation needs to support?
