Remove or correct the "sign in" and "continue as guest" tabs from the Appointment booking page.

The Appointment FTUE is not fully applied, it should not have the tabs with sign in or continue as guest for booking. It should just have a form with Name and Email to be filled out.

I would recommend the following approach:

• if Update Booker's page with new hi-fi designs #1285 is not a lot of work, implement that for the community release instead
• If it is, remove "Continue as Guest" and "Sign In", the contents of the sign in tab, and rename the button to "Book Appointment". Fix Update Booker's page with new hi-fi designs #1285 for earlybird release.
• If the thing that is more work is the "Add Note", then only defer that to Earlybird
