Rewrite node terminal proxy code

This adds support for:

  - Instances in arbitrary projects (this was supported but not right)
  - Multiple users with multiple tabs on the same instance
  - Checking the status of a process id
  - 5 minute re-connection grace period
   - Users can refresh the page / change devices
