Add a document navigation panel

Branched from feature #2651.

Add a collapsible side panel that can navigate the open document. It could replace the heading navigation dropdown menu in each panel, and have options to include tags, references and comments/notes. Everything needed is available in the index more or less, so it should be straightforward.
