Fix sections and indents.

- "Trust Model" looks like it should be h3.
- "Subject-Holder Relationships" entire section was adjusted to h(n-1).
- "Proof Formats" looks like it was meant to be h3. Also adjust sub-sections.
- Fix extra/missing section and other tags.
- Fix indenting on lots of tags for consistency and easier readability.
