Use chosen-sortable in CDR Export fieds input

CDR export form saving fieds array incorrectly ordered. We have to use:

input_html: { class: 'chosen-sortable', multiple: true },

to fix this

chosen-sortable was enabled and now it preserve fields order when fields selected manually in form, but there is still issues with default fields generated by backend:

• when Copy button used
• when New button used, application should copy fields from latest export with type "Base"
