Feature Req: better session management

Would be a much better user experience to separate each Claude session into a separate Discord thread. Kimaki-Opencode does this:

Since discord uses -> - /thread name: message:

I set it to -> - /claude-thread name: prompt:

also included a fix now each thread or chat made continues the session instead of making a new one each time /claude prompt is sendt

1 thread - 1 session which keeps context not spawning a new session
