Feature Request - More Modern UI - Edits Included

Hello. I love the functionality of HRConvert2.... but being a very visual design person, I feel the web UI needs an upgrade.

I have made some local edits and came up with the below:

I have forked your repo and added the changes. I am not Mr. Programmer so please forgive any coding faux-pas.
