Bluetooth: Host: Don't invoke callbacks from tx_work

We are doing TX callbacks from tx_work if the connection is disconnected or a bt_send error occurs. We should get rid of that and consolidate the callback invocations on different thread.

The TX callbacks in question should be processed in the same way as normal.
