fix: Notification handling for active routines and limit warnings

Bug Description

• For limit warnings Reef starts spamming notifications each time an accessibility event is detected on the app to be blocked.
• For active routines Reef notifies that a Routine is active every few minutes.

Expected Behavior

• Both limit warnings and active routines are only notified once. Limit warnings when there is less than a minute of usage left and Active routines only on start time.
