Merge docs/vscode-changelog for v3.2.0 release notes

Brings the architect-curated release notes (docs/releases/UNRELEASED.md +
packages/vscode/CHANGELOG.md [Unreleased]) into main for the v3.2.0 Karnak
release. Resolves the [Unreleased] superset (5 What's new + 6 Bug fixes)
against the three builder-added bullets already on main from PRs #1031 / #1052.
