Add missing output for reasoning effort, updated evals including o3-mini

Purpose

This PR adds:

• Missing output for reasoning effort - my local environment was showing null due to the missing env variable
• Added an evaluation for o3-mini
• Redid the other evaluations with gpt-4o as the judge (previous ones were with gpt-4 turbo, which is deprecated/old)

Does this introduce a breaking change?

When developers merge from main and run the server, azd up, or azd deploy, will this produce an error?
