Adding locked status for appointment

Adding locked status for appointment.
We need locked status as an intermediate later to book an appointment for a patient who would like to do an online payment.
Once the payment is successful, the appointment can be marked as Scheduled.

I think some community inputs are required before this requirement can be merged.

Also a JIRA issue is needed for every code/PR. Please create a JIRA issue, with issue description, and update the PR/Commit messages with JIRA Number. This is required for traceability purposes.
