What Does This PR Do?
Updates v3 diagnosis FML mapping logic to remove any deduplication of diagnoses.
Updates cardinality for supportinginfo sequence on items, to align with the FHIR cardinality (we'll take advantage of this if we eliminate more extensions on item).
Renames item-institutional to item -> this is more correct and has bugged me for a while, since it's a relic of when this mapping only handled institutional claims. We expanded it to handle professional claims, too.
