Upgrade to 4.0.0

This release is not backward compatible with 3.x branch so if you want to upgrade your project you need to do the following things:

• run rake lipsiadmin:update:javascripts
• remove from all uploaded files with our “paperclip” from basename the word “original_”
• update your attachments table removing from the content attachment_file_name column the word “original_”
• check all open_window helper and replace open_grid or open_form according to our rdoc.
