This work adds visual examples for variant 2 of the Ask for user feedback pattern.
There is no coded example for the approach taken in Early Years Training (yet), so @chindle-education1 has kindly taken screen shots of one example feedback survey at the end of a module. I (@SteveMilnes) have added those to the page, with appropriate alt text.
The intention is that this helps to communicate the approach to designers who might use it, in the spirit of showing as well as telling.
The PR contains 5 screenshots, the before and after used here to demonstrate the change only use 2.
If we think 5 screen shots if overkill, then happy to remove some before we merge.
