feat: enhance database / app / compose form with app name auto-generation

This PR enhances the database / application / compose input form by implementing several UX improvements:

• Added app name auto-generation based on the Name field input
• Added lock/unlock mechanism for "App Name" editing
• Reordered form fields to position server selection appropriately under app configuration
• Added protection against password manager auto-fill for the field

• Confirmed tooltip displays properly
• Tested form submission with both auto-generated and manually edited app names
• Verified server selection placement is correctly positioned
