Reworked HTTP handling to align with the transports provided by mcp, adding a Streamable HTTP server path (/mcp) and updating the SSE-based transport (/sse + /messages/) with optional CORS.
Added graceful shutdown handling so Ctrl+C exits cleanly without a traceback.
Expanded README to document how to run each transport, the endpoints they expose, and example mcp-remote configurations.
