docs: Simplify AGENTS.md and add Claude skills

Simplifies AGENTS.md (based on my on sense and the advice of the robot itself) and extracts some if the advice therein into skills. Also adds a .claude/settings.json that make these skills allowed by default, and denies reading from .env files.

• Adds Claude skills: code-review and glossary with succinct instructions for use
• Introduces .claude/settings.json to allow these skills by default and explicitly deny reading .env files
