This has been changed to fit in with the prototype
This enables the header to be updated to show the active tab, as per the prototype.

Description
Some header/nav fixes from snagging. Reference the prototype
Service name should show to the right of the NHS logo. This is probably after we updated the header component. Might be a parameter name change name -> title or something like that
Add the Provider name to the left of the logged in user
The 'clinics' link in the nav should say 'Screening' and should be active (white bottom border) on all of the mammogram appointment pages and clinic list
