Description

Updated JMeter files that are intended to run with the new workflow and run as file driven. The scripts should use a cohort file for upload, consent and vaccinations.

This PR is just for consent and vaccinations, in order to test the workflow. Further changes will be required.

A cohort file is uploaded with this PR purely as a test process, ultimately the cohort files will be generated as part of the workflow.

Context

This is the next step towards fully automating and repeatable performance tests
