CCM-11474: message plan name screen

Description

Adds page to create a message plan. User can enter the plan name and select a campaign id.
If only 1 campaign id on the client, then it just displays the campaign id and doesn't show the <select> field.
On submitting the form, a message plan is created in the backend and user is redirected to the template selection page.
If client has no campaigns, redirects to an invalid config page
If the pages is accessed with missing / invalid messageOrder query parameter, the user is redirected back to the "choose message order" page.
