Adds server implementation

This adds a server implementation with generic backend data stores and Vapor compatibility.

There were a large number of associated improvements to the package, including:

• Adding Filter parsing and processing support
• Adding a generic API protocol for easier alignment between client & server
• Moving HisItem & HisReadRange to the base package for sharing between client and server code
• Adding Swift collection support to collection types
• Adding new Grid convenience methods
• And more!

This is a breaking change
