Capture telemetry for usage of code actions

Add telemetry capture for code and terminal actions in ClineProvider using TelemetryService.

• Telemetry:
  • Add CODE_ACTION_USED event to PostHogClient.EVENTS.TASK in TelemetryService.ts.
  • Implement captureCodeActionUsed() in TelemetryService to log code action usage.
• ClineProvider:
  • Capture telemetry for code actions in handleCodeAction().
  • Capture telemetry for terminal actions in handleTerminalAction().
