Profile-scoped MCP servers are now properly removed from user, local, and project scopes via "claude mcp remove" before being configured through create_mcp_config_file().
Additionally, stale {command}-mcp.json files are now deleted when no profile servers are configured, preventing leftover config files when switching configurations.
