Update remote-mcp-server.mdx to address various errors

All changes are in the Auth section of the article

Specifics

• Update port from 8787 to 8788 to match with example code
• Revised instruction sequence to avoid deploying before values are configured
• Add new instructions to match the updated OAuth flow in the MCP Inspector
• Add Cookie generation
• Add KV generation
