Updated steps for cert creation to make command example.

Step 2 of the 'Create a CA Chain File' states to upload the contents of ca.pem, but if following the command examples, the CA chain file will be called ca-chain.pem.

Updated steps so the name is the same in both the command example and step for uploading to Cloudflare
