Proxy - Scheme Support

Added Scheme Support for automatic callback link behind proxy (via HTTP_X_FORWARDED_PROTO http header).

Initial Release

Zip file to upload in Limesurvey.

Pre-release because I just tested it myself, don't want to say its fully production ready, even tho I use it in production myself.
