Remove the refresh button/command

This PR removes the refresh button and command functionality from the Doc Detective VS Code extension as it's no longer needed since automatic refreshing was implemented.

Changes made:

• Removed the refresh command registration from extension.ts
• Removed the refresh button from the UI by deleting its entry in package.json
• Removed the activation event for the refresh command in package.json
• Updated tests to remove assertions related to the refresh command
• Removed code that handles refresh messages from the webview

These changes make the UI cleaner and remove unnecessary code since the extension now automatically refreshes when files are saved, editors change, or configuration is updated.

Fixes #4.
