Replace repository with a breadcrumb repo+branch in heading of active item

Description

Resolves part of #4332

Replaces repository with a breadcrumb that has both the repository and current branch

• uses the repository icon to the left of the repository name instead of the hosting service icon
• adds a breadcrumb to display the current branch
• hides the repository name by default and reveal it when the repo icon is clicked or hovered
• truncates the branch name when there isn't room to display it
