based on top of @XeroXer work in #3

Additionally:

  • support comments on tables and columns
  • adds blob type
  • adds fulltext index
  • fix "no primary key" (when using "named primary")
  • keep column collation if different from table
  • better limit parsing
  • blacklist the phinxlog table
