docs: document forge-std version requirements

Description:

This PR documents the minimum version needed for forge-std dependency. It also includes a new section in FAQ so it's easier for users to troubleshoot when they encounter this issue.

Moreover, it adds a note in the README about including --skip-simulation when running forge script.
