AU eRequesting profile updates

New profiles were added to AU eRequesting as part of ballot reconciliation: Organization, PractitionerRole, Practitioner, Encounter, Patient

There are 2 types of change in this PR:

1. Add the new profile only to meta.profile and ensure resource instance is in au-erequesting folder (where the existing instance is not used in other au core references)
2. Add the new profile to an existing instance in the au-core folder (Where the existing instance is already used in au-core references)

There is also 1 new Encounter profile : Encounter-gpvisit

eReq PractitionerRole.practitioner has type of eReq Practitioner, can we also add eReq profile to this Practitioner in PractitionerRole-generalpractitioner-guthridge-jarred.json?

Updates:

1. Added eRequesting Practitioner to PractitionerRole-generalpractitioner-guthridge-jarred.json
2. Moved Patient-scott-elijah-ken.json to eRequesting folder and changed profile to AU eRequesting Patient
3. Added AU eRequesting Patient to AU Core Patient: mackay-fritz
