EOD v1.5 Review and Conversion

In the audit log below, a few of the changes impact how a case should be coded. We recommend the following:

1. If Schema ID [#3800] = 00360 (Lung)

   • If EOD Primary Tumor [#772] = 550, convert to EOD Primary Tumor = 675 (The value 550 was not reused so this conversion can always be applied.)

2. If Schema ID [#3800] = 00580 (Prostate)

   • If Prostate Pathological Extension [#3919] = 250, convert to Prostate Pathological Extension = 300 (The value 250 was not reused so this conversion can always be applied)

3. If Schema ID [#3800] = 00740 (Thyroid Medullary)

   When you first deploy version 1.5 if you have existing data, apply the conversion

   • If EOD Primary Tumor [#772] = 300, convert to EOD Primary Tumor = 400
   • If EOD Primary Tumor = 200, convert to EOD Primary Tumor = 300

   Moving forward, for databases where new records are imported from outside sources (such as a central registry), require a review

   • If EOD Primary Tumor = 200 or 300, please review

4. These two combinations must be flagged as needing review. Since the schema changed, the associated SSDIs and valid values for schema specific fields would also change.

   • If primary site = C210 and histology = 8720-8790, please review (Schema changed from Melanoma Skin (00470) to Anus (00210))
   • If primary site = C809 and histology = 8041, please review (Schema changed from Merkel Cell Skin (00460) to Ill-Defined Other (99999))

EOD v1.5 changes

  • Grade Clinical and Grade Pathological: Note was added that for bladder, TURB does not qualify for surgical resection and is considered clinical only

• Brain, CNS Other, Intracranial Gland and Orbital Sarcoma, Soft Tissue Head and Neck, Soft Tissue Other:

  • Sites C700-C729, C751-C753 with histologies 9380-9539 were moved for all behaviors (0, 1 and 3) from sacroma schemas into the brain related schemas. This EXCLUDES any combinations specifically mentioned by AJCC as eligible for staging in the sacroma chapters.

• Colon and Rectum:

  • KRAS: Note 4 and 5 were added to describe where information can be gathered from
  • Perineural Invasion: Note 3 was modified to clarify where information can be obtained from
  • Microsatellite Instability: Note 2 (describing what MSI testing is) and Note 3 (describing how such testing is done) were modified. Note 4 (about terms used in Canada) was removed.

• Cervical Lymph Nodes and Unknown Primary Tumor of the Head and Neck: Schema Discriminator 1 Notes were revised to improve clarity regarding how this item is to be assigned

• Cutaneous Squamous Cell Carcinoma of Head and Neck:

  • The schema name was changed to be Cutaneous Carcinoma of Head and Neck
  • References to Chapter 15 (Cutaneous Carcinoma of Head and Neck) were changed to remove "Squamous Cell" from chapter name
  • The determination of EOD Derived T based on Tumor Size was updated to synchronize with AJCC changes from 2/2/2018 to T1, T2 and T3

  • Bilirubin Pretreatment Total Lab Values Note 3 was revised to indicate that Total Lab Values include conjugated, unconjugated and total bilirubin values
  • Creatinine Pretreatment Unit of Measure [3825] was changed to reflect that is is only required by SEER if CoC collects the case (as is lab value)

• Lymphoma CLL/SLL: Anemia Note 3 was revised to include hemoglobin & hematocrit, H&H as test types

• Melanoma Skin: EOD Regional Nodes code 700 was updated to provide more information for two or more clinically occult or detected nodes

• Oral Cavity (Chapter 7) related schemas:

  • References to Chapter 7 (Oral Cavity) were changed to remove "Lip and" from chapter name
  • The determination of EOD Derived T based on EOD Primary Tumor and Depth of Invasion was updated to synchronize with AJCC changes from 5/25/2018 related to T1, T2, T3 and T4a

• Ovary:

  • EOD Primary Tumor, Note 6 exceptions to large intestine were changed to include "(except rectum, rectosigmoid and sigmoid colon)"
  • EOD Primary Tumor, Code 750 modified to remove requirement of microscopic confirmation

• Plasma Cell Myeloma and Plasma Cell Disorders: The AJCC ID was changed so that Plasma Cell Myeloma is 82.1 and Plasma Cell Disorders is 82.2

• Primary Cutaneous Lymphoma: EOD Primary Tumor codes 400 and 500 were added to address the addition of T3a and T3b to AJCC T (2/2/2018)

• Prostate:

  • Prostate Path Extension [3919] is no longer flagged as an SSDI. This is an EOD field.
  • Grade Clinical and Grade Pathological: Note added to indicate that TURP does not qualify for surgical resection and is considered clinical only

• Thyroid: EOD Regional Nodes, code 300 was modified to add Level VII-Superior mediastinal group

• General

  • Grade Clinical: Note about only one grade available was revised. If cannot determine if clinical or pathological, assume it is clinical and code appropriately per clinical grade categories.
  • Grade Pathological: Note about highest grade was revised to clarify statement about using clinical grade if it is the highest. This follows the AJCC rule that pathological time frame includes all clinical time frame information plus information from the resected specimen. Example situations provided where resection is done but either no grade is documented or there was no residual cancer.
  • Grade Post Therapy: Note about Code 9 had bullet added for "Surgical resection is done after neoadjuvant therapy and there is no residual cancer"
  • Adjustments were made to calculation tables to handle DCO cases not having SSDI values
  • Adjusted NPCR required fields to match the Updated REQ Data Item List for Summ Stage Only (EXCEL) from the "Update from CDC-NPCR on 2018 Changes" as sent by Vicki Benard
  • Tumor Size Clinical, Pathological, and Summary: for those schema where 999 is the only valid value, the lookup was limited to 999. 998 was removed from schemas where it does not apply.
  • LVI was added to all schema. Prostate and Testis lookup does not include 8. Schemas that can only be 8 have a lookup with only that value.
  • Searched and replaced quotes and dashes from Microsoft Word with standard characters

CS Algorithm Bug Fix

Fixed Myeloma Plasma Cell Disorders schema. The default values for the following were removed:

• CS Lymph Nodes
• SSF2 - Durie-Salmon Staging System
• SSF3 - Multiple Myeloma Terminology

Previously these values had been footnote indicators rather than valid values. (#5)

STAGING_ERROR column information

Staging library now returns the list of columns involved in staging errors.

This adds a new field to the Error class.

List<String> _columns

It is set in two situations:

1. If there is a match to an ERROR cell, the list will contain a single element which represents the column property of the endpoint.
2. If there is no match found in any row (i.e. MATCH_NOT_FOUND), all the endpoint column properties will be added to the list.

EOD 1.3

The EOD algorithm was updated to version 1.3.

Breast

• Estrogen Receptor Total Allred Score [#3828] and Progesterone Receptor Total Allred Score [#3916] status for NPRC changed to not required. These are required if available only.
• Note added to Tumor Size validation tables noting exception to the rounding rules for sizes under 2 mm.

Brain, CNS Other, Intracranial Gland

• Schema assignment for C700-C729, C751-C753 for all histologies with behavior = 0 or 1 changed to be these schemas. AJCC Chapter Calculation tables updated to include these combinations. Recommend all C700-C729, C751-C753 with behavior = 0 and 1 be re-examined as schema may have changed. Also, Behavior = 2 was removed from the AJCC Chapter Calculation tables for these schema as it is invalid with these site codes.

Cervical Lymph Nodes and Unknown Primary Tumor of the Head and Neck

• Continuation of v1.2 change, AJCC Chapter Calculation changed to not allow behavior = 2 for ANY histology (v1.2 only changed histologies related to AJCC Chapter) as ‘in situ’ and ‘lymph nodes positive’ are conflicting states.

Colon

• Microsatellite Instability (MSI) [#3890] status for NPRC changed to not required. This is required if available only.

Ill-Defined Other

• Summary Stage 2018, Note 5 was modified by having '5' removed. 5 is no longer a valid code in any Summary Stage chapter.

Lymphoma CLL/SLL

• Fixed name for NCCN International Prognostic Index (IPI) [#3896]

Prostate

• Number of Cores Positive [#3898] code of "00- All examined cores negative" was added.

Testis

• S Category Clinical [#3923] and S Category Pathological [#3924] were changed to be required by NPCR.

General

• SSDIs that are shown in NAACCR Volume II, Chapter VIII as being RC for SEER were changed to not be required by SEER.
• Schema and SS2018 both reference the Summary Stage 2000 manual. The website for the manual was updated.

Updated TNM algorithm to 1.9.

• Bone: An error that was introduced into the calculation for Combined N in version 1.8 was corrected.
• Epiglottis Anterior: Note 3 Summary Stage 2000 chapter was corrected to be Tonsil, Oropharynx on page 60.
• General: The Combined N logic when Clin N = cX, Path N = pX and Reg Nodes Positive = 00, was to set Combined N to 0. It was adjusted to include the c prefix for a final value of c0.

Algorithm Updates

Updated EOD algorithm to 1.2.

Bladder

• Grade fields: a Note was added describing the priority order.
• EOD Primary Site: Note 3, 1st bullet, the code associated with "noninvasive" was changed to 000
• EOD Primary Site: Note 3, 3rd bullet, the code associated with "confined to mucosa, NOS" was changed to 100
• SS2018: Note 5, 3rd bullet, the code associated with "confined to mucosa, NOS" was changed to L .

Breast

• AJCC Chapter Calculation was re-aligned to the AJCC histology list. Only a few histologies are AJCC eligible with behavior = 2.

Cervical Lymph Nodes and Unknown Primary

• AJCC Chapter Calculation changed to not allow behavior = 2 as ‘in situ’ and ‘lymph nodes positive’ are conflicting states.

Esophagus and Esophagus Squamous

• AJCC Chapter Calculation changed so that 8020 is AJCC eligible with behavior=2. Behavior was removed from the calculation in these schema.

Hypopharynx: EOD Primary Tumor codes

• Code 300 changed to "Fixation of hemilarynx or larynx"
• Code 400 changed "Fixation of hemilarynx, larynx or oropharynx" to be "Fixation of oropharynx"

Larynx Glottic

• Summary Stage code 1 indenting was corrected. Everything after ‘Adjacent region(s) of larynx involved’ is a subcategory of that.

Vagina

• Order of SSDIs was changed to match the order in Cervix (LN Distant: Mediastinal, Scalene moved down.)

General

• Schema names and short names were reviewed and revised to be consistent.

TNM 1.7

The TNM version was updated from 1.6 to 1.7.

• Colon and Rectum, Combined T table: When one of Clinical T or Path T is missing, the Combined T table for Colon and Rectum was not handling T4. T4 has been added.
• General: if Clin and Path T, N, M and Stage Group are all set to 88, then the Derived Combined T, N, M and Stage Group will be set to 88. This happened without error for most schemas, but Prostate was incorrectly returning an error messages.

A bug was corrected where the internal context was being returned when it should not have been.
