Sure, that's weird they're broken now, I will take a look next week. I must have messed up something in the command-line signature but I'm not sure how. |

This PR adds support for including a Table of Contents as the first page of a PDF, addressing #12 and possibly #91. Please note it's a pretty major change to how the subprocess is called, so I understand if this is a non-starter, but I wanted to propose it so users wanting support for
toccan find it. Working for me on OSX and Windows.
