In distributed inference, we have DP == data parallel, and P/D == Prefill/Decode Disaggregation.
The inference scheduler will need to deal with both of these concepts. This PR renames
docs/dp.mdtodocs/pd_disagg.mdfor consistency and to avoid confusion.
