Move WithResponseDescription and WithLocationHeader from OpenApiExtensions.cs to a new RouteHandlerBuilderExtensions.cs for better organization. Add ProducesDefaultProblem method to simplify adding multiple ProblemDetails responses. Improves discoverability and maintainability of RouteHandlerBuilder-related OpenAPI extensions.
