The default font contains superscript glyphs for numeric characters, which means that superscript `[1]` will look larger than superscript `[1, 2]` or `[1, #sym.ast]`. Therefore, all superscripts in affiliations should be synthesized for consistency. Also simplify logic for affiliation superscript generation and remove thin space between author name and affiliations if neither an affiliation is provided or the corresponding author flag is set.

Fix typos in the manual (
institutionsshould beaffiliations) and disable superscript glyphs in author affiliations as they clash with synthesized glyphs.Apologies for whitespace modifications, the files use DOS line endings and I use Unix EOLs.
