Bumps Helm chart to 0.1.47 (appVersion v1.2.1) and updates CHANGELOG with entries for 0.1.45–0.1.47 reflecting component version bumps.

• Helm chart:
  • Update charts/plane/chart/Chart.yaml version to 0.1.47 and appVersion to v1.2.1.
• Changelog:
  • Append CHANGELOG.md entries for 0.1.45–0.1.47, documenting upgrades of plane-backend, plane-frontend, and plane-space up to v1.2.1.
